Intake
To begin the intake process, please call or e-mail us and set up a time to do an intake interview. An intake interview can be done over the phone, at our office, or in your home. This initial conversation will help us learn more about your child. We can also discuss our different services and see if our program meets your needs.
Once the intake interview has been done, it is time to set up an appointment for your child to meet with our Psychologist to do an Initial Evaluation. The goal of this evaluation is to confirm the diagnosis and determine appropriate treatment goals for your child. ACT offers a variety of testing services for children. Please contact us with questions about our testing or to set up an appointment.

Funding
The Children's Long Term Support Waiver: The CLTS waiver is a Medicaid waiver program in the State of Wisconsin that covers intensive therapy services for up to 3 years for children who qualify for these services. ACT is recognized as an accepted provider in many counties throughout the state. There are many steps in the process of applying for and securing a "waiver slot." Please contact the Wisconsin Department of Health Services at dhs.wisconsin.gov for information about how to begin the application process.
Insurance: Due to an insurance mandate in the state of Wisconsin, many children have access to treatment that is covered by their insurance plans. Coverage varies by plan and services are subject to deductibles, co-pays, and co-insurance fees. ACT contracts with several insurance companies. Contact your insurance provider to learn if your plan covers treatment. Ask what providers are covered in your area.
